 Session Rates

  • Initial (intake) Session: $200
  • Individual Play Therapy Session 50 min: $195
  • Parent or Caregiver Consultation/Family Psychotherapy Session: $175
  • School Observation with Full Report $200
  • Walk and Talk Therapy $150


Insurance for Illinois

Play It Forward Counseling accepts the following insurance plans:

Blue Cross Blue Shield, Aetna, Cigna, and United Healthcare.


Out of Network Provider:  Play It Forward Counseling can provide you with a “superbill” in order to file an out-of-network claim for reimbursement based on your insurance plan’s benefits.  The superbill is a detailed receipt of your payment for services that provides all of the information necessary for an out of network claim.


Private Pay Information

While there are benefits to using health insurance to pay for mental health services, there are significant drawbacks. These drawbacks include:

1. Health insurance companies require a diagnosis in order to pay for services 

2. The diagnosis becomes part of your child’s medical record

 3. Health insurance companies have complete access to your child’s confidential record therefore, complete privacy cannot be guaranteed 

4. Health insurance companies can determine treatment details such as frequency and the number of sessions. 



Payment

Payment is due at the beginning of each session. Accepted forms of payment include credit cards, debit cards, cash, check, PayPal, Venmo and Health Spending Account (HSA) and Flexible Spending Account (FSA) cards. 


Sliding Scale

I strongly believe that therapy should be available to everyone, and I strive to make counseling affordable. If finances are a barrier to your family seeking help, a sliding scale fee is available on a limited basis. 


Cancellation/ No Show Policy

Life happens, and you may need to cancel or change your appointment. If you cancel your appointment with at least 24 hours notice, there will be no fee. Appointments cancelled less than 24 hours in advance will be charged a fee of $75. No call/ No show appointments will be charged a fee of $150. If you cancel or fail to show for 3 consecutive appointments, services will be terminated and you will be provided with a referral to other providers in the area. 


No Suprise Act/ Good Faith Payment

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. 
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. 
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. 
  • Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises
